Thank you for choosing our office for your oral health needs.
We hope that as you visit our office, you will come to experience our philosophy, which is to treat
patients with a caring attitude, provide clinical excellence, and to keep service a top priority.
Due to the COVID-19 Pandemic, we have made some changes as per the Center for Disease Control and
American Dental Association guidelines to help protect our patients and staff. Please read the following carefully:

Prior to the appointment:

Our office will ask you to complete some screening questions relating to COVID risk and obtain
information about your oral condition and your medical history. You’ll be asked some screening
questions again when you are in the office, to ensure that any new changes are communicated.

On the day of the appointment:

• Please wear a mask before you enter the office
• We have hand sanitizer that we will ask you to use when you enter the office, and we will check
  your temperature at the office
• If possible, we ask that the patient come in alone for the appointment. If a parent or guardian is
  required, we ask that just one person accompany the patient and as possible wait outside the office
  during the visit.

Scheduling:

We are limiting patient appointments to allow for social distancing and sufficient time for surface
disinfection and air filtration/disinfection between patients.

Office modifications and procedures undertaken for enhanced air filtration/disinfection and control of
transmission of pathogens during COVID-19 Pandemic:

• Enhanced surface disinfection.
• Use of extra-oral high-volume suction machine with HEPA filters and internal UVC light sanitization of air during and following procedures and as part of room air disinfection.
  Air purifiers using HEPA + UVC light for disinfection
• HVAC system upgrades including installation of HEPA filters
• Plexiglas barriers at the check-in desks
• Appropriate PPE availability and use
